If you are experiencing a mental health emergency or are in immediate distress, please do not use this form. Instead, call 911 or go to your nearest emergency room for urgent care. Our team reviews inquiries during business hours and will respond within 48 hours, but this form is not monitored for crisis situations.

Cash pay rates $300 for psychiatric evaluation, $150 for followup appointments, Sliding Scale offered on individual basis
